Link Worker - Social Prescribing Project

Job Posting: Link Worker for Social Prescribing Project (Full-time temporary position October 2024 to June 2026; Location: Labrador-Grenfell, St. Anthony Family Care Team.

About the Link Worker for Social Prescribing:
The Link Worker position is the first of its kind in Newfoundland and Labrador. As a member of the Family Care Team (FCT), the link worker will play a critical role in social prescribing within the NL health care system. The Link Worker will work with older adults aged 50+ who have been referred by a member of a FCT.
The position will contribute to older adults’ health and well-being by assessing their needs and connecting them to a range of non-clinical community supports. These could include programs and services that can help with managing stress and loneliness, finding social groups, and engaging in physical activity. The position will also assist with referring and providing information on employment options, various benefits specific to the individual, housing programs, and legal services in the region.
Vital skills for this role are the ability to build a strong relationship with the FCT and to establish quick rapport with the people being referred by listening carefully to what is important to them and what motivates them.

This position requires a person who is respectful of others, well organized, efficient, detail-oriented, and can work in a collaborative environment. Specific duties necessary for this position include, but are not limited to:
  • Deliver effective social prescribing referrals as identified with individuals and from the perspective of “what matters to you.”
  • Work as a collaborative member of the FCT.
  • Contribute to the evaluation of the service by collecting and recording monitoring, impact, and outcome data using required systems.
  • Manage own caseload to ensure individuals receive appropriate levels of support and that expectations are met.
  • Act as a mentor to new Link Workers using the training given and the experience gained.
  • Maintain thorough documentation and ensure follow-up on important actions and decisions.
  • If required, attend and participate in stakeholder meetings/workshops and team meetings.
Skills and Qualifications:
  • Experience in a role that involves delivering face-to-face support in a relevant health or social care environment. This can be acquired through a combination of post-secondary education in a human services field, work, and/or extended volunteer experience.
  • Experience working with populations facing multiple barriers to accessible and equitable supports.
  • Experience working with older adults.
  • Experience in the areas of population health and health equity.
  • Understanding of the social determinants of health.
  • An understanding of the principles of privacy and confidentiality.
  • Ability to think strategically and analytically.
  • Ability to work well independently and in a team environment.
  • Understanding of and ability to collect and record data to monitor key performance indicators.
  • Excellent time management skills.
  • Ability to adapt to changing environments and timelines.
  • Excellent communication and interpersonal skills.
  • Proficiency with Microsoft products, applications, and services.
  • If hired, a certificate of conduct and a vulnerable person sector check is required.
  • Access to a vehicle, as travel will be required.
Compensation:
$50,000 annually, with a 3% increase in years two and three. Three weeks vacation annually. Monday to Friday schedule. Flexible work model.
