Inclusion NB is seeking a dynamic, energetic and enthusiastic individual committed to the vision, values and philosophy of inclusion. The Adult Life Transition Facilitator will plan and collaborate with youth who have a significant disability on creating a successful transition from high school to adult life. The Facilitator will also support and build capacity within high school personnel and educators to better support youth with significant disabilities to be prepared for life after high school. This posting is to cover a maternity leave position until June 30, 2025. Mission: Inclusion NB is leading the creation of opportunities for individuals with an intellectual or developmental disability and their families to live full and valued lives in all aspects of society. Responsibilities:
Assist students and families leaving high school in the next two years in identifying goals and interests for life after high school;
Co-develop concrete and clear post high school plans for youth with significant disabilities;
Support youth in high school who have significant disabilities to learn about adult life goals and to develop decision-making skills;
Support and build capacity within families to meaningfully participate in planning for post-school life for their youth who have significant disabilities;
Collaborate, support, and build capacity within high schools to better support youth with significant disabilities to be prepared for life after high school;
Ensure youth with significant disabilities have seamless supports in the transition from high school to adult life;
Enable and support student self-determination;
Ensure effective links and collaborations with government and community support programs are in place to support post high school adult life goals and options for youth with significant disabilities.
In addition, assist individual in the identification of life goals, including:
Further education/training/skill development
Community involvement
Homelife
Paid employment
Relationships
Engage with communities to continue identifying partners and opportunities for students who have significant disabilities;
Access government funding for support and services for the student.
Qualifications:
A bachelor’s degree in counselling, career development, or social sciences;
A bachelor’s degree in Education is considered a strong asset;
Strong computer skills, ability to use Microsoft Office applications, comprehensive internet searches, and the ability to learn new information management systems;
Previous work experience working with students and individuals with an intellectual or developmental disability is considered an asset;
Understanding of career development is considered an asset; The ability to travel and provide own transportation.
