Key Responsibilities Personal Care and Assistance: Deliver hands-on support with essential daily activities such as personal hygiene, dressing, feeding, and mobility. Prioritize respecting individual preferences, promoting autonomy, and encouraging self-care skills wherever possible to enhance independence. Emotional and Social Support: Create a nurturing and positive environment by building genuine, supportive relationships with individuals. Offer companionship, listen actively, and provide emotional encouragement. Support participation in social activities and recreational pursuits to build a sense of connection and well-being. Care Planning and Documentation: Participate in the development and implementation of personalized care plans tailored to each individual’s needs and goals. Accurately record and document care activities, progress, and any changes, ensuring timely communication with relevant team members to support coordinated care. Community Integration: Encourage and assist individuals in accessing and participating in community events, activities, and social outings. Foster social interaction and inclusion to strengthen community ties and promote a more enriched and inclusive life experience. Medication Management: Administer prescribed medications in line with established protocols, ensuring safety and accuracy in all aspects of medication handling. Monitor for potential side effects or changes in health and promptly report any observations to relevant team members. Advocacy and Empowerment: Champion the rights, choices, and dignity of individuals with disabilities. Empower them to make informed decisions and actively participate in their care planning, promoting a sense of agency in daily routines and life activities. Collaborative Teamwork: Collaborate closely with a multidisciplinary team that includes healthcare providers, families, and external service providers. Foster open communication and a unified approach to ensure seamless and comprehensive care delivery. Qualifications and Skills:
Current Australian Driver’ license with full comprehensive insurance and willing to transport clients in your own vehicle.
Current First Aid + CPR certificate.
Current DHS (DCSI) Disability Employment Screening Clearance/NDIS Worker Check or willing to complete one.
(WWCC) Working with Children Check
Proof of COVID19 vaccination preferable.
Previous experience in disability support work or a related field, particularly with complex behaviour support, is preferred.
Relevant qualifications in disability support, individual support, or a related field.
