Identifying client needs and creating, developing, and implementing customized educational, training, and support plans.
Interviewing clients to assess the severity and nature of their challenges.
Tracking and documenting client progress.
Directing clients to external organizations for additional assistance.
Evaluating community needs and available resources for services like health, welfare, housing, employment, and training.
Working with community groups, welfare organizations, government agencies, and businesses to address local issues and raise awareness of available services.
Providing care and education for children and individuals with disabilities in adult care facilities, group homes, and government institutions.
Overseeing individuals on probation or parole.
Helping young people resolve social, emotional, and financial issues.
Drafting funding applications, resource requests, and reports for government and external agencies.
Supports the efforts of Social Workers and Welfare Workers by delivering services and assistance to families.
Relevant Qualifications and Skills to be eligible for this role At least certificate III or diploma in community services or similar qualifications. At least 2 years of industry experience as a Family Support Worker or preferably in disability, mental health, or aged care settings. Ability to develop and maintain professional, supportive relationships with clients. Strong knowledge of the NDIS and disabilities in Australia. Current First Aid certificate. Manual Handling certificate. Current Australian Driver's Licence (preferred). Good customer service skills. Excellent time management skills. Proficient in administrative and case management skills.
