About the role We are looking for Disability support workers in Perth who have good availability to cover several shifts i.e., Mornings, afternoons, Nights and sleepover shifts. Please state in your application if you prefer days or nights. Candidates must be comfortable working with dog/pets around them. This is a challenging but rewarding opportunity for you to make a positive change in the lives of People with a physical and Mental Disability. Due to growth, we are looking for a team of experienced Disability Support Workers to provide support to our clients all over in Perth region. Working in a team, you will facilitate the delivery of quality services and achieve positive outcomes with service users in a positive and proactive environment. You will be actively supporting their individual needs and creating a safe environment. Key Responsibilities
Support participants to work towards achieving their individual goals.
Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, community access etc.
Support participants in building their confidence.
Liaise and collaborate with clients, their community and community & disability support agencies.
Monitor and review the participant's progress through the service and against the support plan.
Utilize positive behaviour strategies
Support clients to participate in age-appropriate activities and tasks, including employment, in-home disability support and community access.
Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality.
Monitor the safety and well-being of people with disability and report any concerns to the Manager.
Ensure completion of appropriate documentation. As required, relevant information on disability support will be recorded in client notes and communication books.
Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
Liaise extensively with other staff to ensure the overall smooth and consistent running of the disability services.
Report regularly to the Manager
Skills & Experience
Qualification preferably in community/disability/individual support/aged care/ Mental Health and/or preferably with relevant experience.
Experience working with clients with challenging behaviours.
Well-developed verbal and written communication skills.
Current Driver Licence and reliable vehicle.

Valid visa/working rights in Australia
