Your role at St John of God Murdoch Hospital
You will develop, promote, implement and evaluate Day Hospice services and programs to meet the needs of people with advanced life limiting illnesses, and to extend these services to inpatients.
The Position
-
Assist the Leadership team with service planning for Day Hospice Services in response to strategic plans for the Hospice.

Coordinate the day-to-day operations of the Day Hospice.

Provide supervision and direction of services provided by the Programs Coordinator and Volunteer Complementary Therapists.

Develop and implement contemporary programs which integrate available technology, including software-based programs, for clients (inpatient and outpatient).

Foster partnerships with statutory and external organisations to enhance Day Hospice Services to address identified needs for palliative care patients.

Evaluate the Day Hospice Services Programs makes recommendations and facilitates improvements as required.

Maintain records and data management in relation to service provision for future planning and service provision.

Provide reports about progress of programs as required.

Ensure services are delivered which meet the Palliative Care Australian Standards and applies evidence-based practice.

Facilitate group work with patients and carers.

Actively participate in all relevant clinical and non-clinical committees.

Coordinate and integrate service provision with those of other departments (within and between the Hospice and Hospital) and effectively communicates with a broad range of internal customers.

Represent and promote the Hospice through participation in relevant palliative care committees, community and professional bodies.

Respond to inpatient referrals by the health care team in accordance with protocol.

Provide assistance to families and carers that optimises quality of life for patients through functional assessments, discharge planning and resource information.

Provide consultation and support to medical, nursing and allied health professionals regarding aspects of planning, implementing and evaluation appropriate care for individual patients which aim to maximise quality of life.

Develop and deliver education and therapeutic advice (within scope of practice), for individuals, groups and other health professionals.

Provide case reports and other information as required.
You will have demonstrated highly developed interpersonal skills with experience in coordinating client focused services and working with groups.
You will hold demonstrated conceptual, analytical and advanced level of report writing skills with well-developed skills in computer information systems relevant to the position.
To succeed you will hold an undergraduate degree in a health related discipline or equivalent with demonstrated experience in a healthcare setting, in the development and implementation of programs for clients who have a progressive illness.

Salary: $99,917 to $103,090 pro rata, plus 11.5% superannuation
