Spanish Speaking Care Manager (Part-Time Per Case Basis)
We are seeking a compassionate and detail-oriented Part-Time Care Manager to provide client-centered care on a per-case basis in the Bronx. The Care Manager will work closely with individuals and their families to coordinate healthcare services, advocate for patient needs, and ensure comprehensive care plans are followed. The ideal candidate will have experience in case management, social work, or healthcare coordination and be able to assess client needs, connect with community resources, and collaborate with healthcare providers.
**Responsibilities:**
- Conduct assessments and develop individualized care plans for clients.
- Coordinate and monitor healthcare services and resources.
- Act as a liaison between clients, family members, and medical professionals.
- Provide ongoing support and advocacy to ensure client needs are met.
- Document and maintain detailed case notes and care progress.
**Qualifications:**
- Previous experience in care management, social work, or healthcare.
- Strong organizational, communication, and problem-solving skills.
- Ability to work independently and manage multiple cases.
- Knowledge of community resources in the Bronx area is a plus.
- Flexible hours based on case requirements.
