Provides interventions with patients having complex social needs, requiring assistance in eligibility determination for social programs and funding sources and qualifying for community assistance from a variety of special funds and agencies. Offers counseling and direct social work services to assist with meeting psychosocial needs of patients and their families. Promotes achievement of patient and family's optimal level of physical and social functioning. Facilitates and coordinates development of a discharge plan of care for high risk patient populations. Receives referrals of individuals who are of an at-risk population from case managers, staff nurses, team members and physicians. Typically reports to a senior case manager. Requires a master's degree in social work, Licensed by State of Alabama and 2-4 years of experience in the field or in a related area.
