Full-time.
Monday - Friday, 8am - 5pm.

About the Role:
As a Social Services Discharge Planner at Columbine Health Services, you will provide essential support to residents and families facing social, emotional, and medical challenges. You will advocate for resident rights and ensure a safe and supportive environment.
Key Responsibilities:
Communicate residents' needs to appropriate persons.
Assess factors affecting residents and their families, educate about community resources, and assist in referrals for safe discharge and continuity of care.
Provide counseling for long-term planning, crisis intervention, and support system strengthening.
Recognize and plan for the unique needs of physically and cognitively disabled, chronically ill, and terminally ill residents.
Empower residents and their loved ones to fully participate in their environment.
Plan and facilitate annual in-services on resident rights for residents and staff.
Partner with Quality Assurance for ongoing assessment of residents' medical and psychosocial issues.
Ensure appropriate placement, level of care, and unit for residents.
Facilitate care conferences and attend resident council meetings.
Maintain confidentiality of all resident information.
Provide information to the Office Manager regarding admits, discharges, and other resident changes.
Document goals, objectives, and progress for each resident accurately.
Ensure Board and Care Plans are complete and reviewed annually.
Participate in facility meetings and recruit and supervise department interns.
Perform other duties as assigned.
What We're Looking For: Experience and Education:
A bachelor's degree in human services with experience in healthcare preferred.
Experience working with the geriatric population is required.
Experience working with people with Alzheimer's and related memory disorders is preferred.

Our Total Rewards package includes a salary range of between $20 - $24/hr. DOE
