Registered Nurse (RN) Case Manager for our Hospice program!
Constellation Hospice is seeking a compassionate, skilled, and experienced hospice Registered Nurse (RN) to work with our hospice program in Maine. We know that RNs who want to work in hospice care have a heart for patients at this important time and have a passion for supporting them and their families through their life transitions.
Territory: This position will cover Norway to Wells and Kennebunk.
Qualifications:
Graduate of an accredited RN program.
Registered Nurse (RN) license in the state of Maine.
In-home Hospice or community care field experience.
Hospice patient care experience is strongly desired.
Must be able to travel within an assigned territory.
Previous experience with Matrix Care is a plus.
Responsibilities:
Provides skilled nursing care including the administering medications and treatments as prescribed in the physician's plan of care for patients in a home setting.
Performs a comprehensive assessment and coordinates the plan of care with all disciplines associated with the patient's needs, including family and primary caregivers.
Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever a change occurs.
Participates in weekly Interdisciplinary Team Meetings, providing updates on patients' physical, emotional, psychosocial, and spiritual status.
Will utilize a computer-based documentation system and submit all required clinical documentation, time records and other required information in the required time frame.
Instructs, orients and supervises other clinical personnel as necessary.
Provides input to Home Health Aide Supervisor as needed for the purpose of hospice aide evaluation.
May act as a liaison with institutions to promote continuity of care as the patient moves from one level of care to another.
May participate in community activities as a representative of the agency.
Participates in weekend and on-call rotation as needed.
May perform other duties as assigned.
