Full-Time | Troy, NY | $37 - $50/hr

What You'll Do
- Patient Assessments - Evaluate patients' physical, mental, and social needs.
- Care Planning - Work with patients, caregivers, and primary medical staff to develop and adjust personalized care plans.
- Direct Care - Handle wound care, infusion therapy, medication administration, and health education.
- Care Coordination - Oversee LPNs and HHAs to ensure smooth, quality care.
- Documentation & Reporting - Keep clear, detailed records and report any significant changes to the healthcare team.
- Active NYS RN License and a valid NYS Driver's License.
- 1+ year of recent experience in geriatric care (home care experience is a plus).
- Hourly pay: $37 - $50
- Full benefits: Health, dental, vision, 401(k), and more.
- Schedule: Monday to Friday, 8 am - 4 pm
