Responsibilities
Pay: $85,000.00 - $150,000.00 per year

Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Utilize EMR and EHR systems to document patient information accurately.
- Code own visits
- Collaborate with healthcare team to ensure optimal patient care and outcomes.
- Educate patients and families on health management and treatment options.
- Valid Nurse Practitioner or Physician Assistant license
- Strong patient assessment, communication and care skills
- Knowledge of lifestyle medicine, motivational interviewing & health coaching helpful.
- Ability to work with diverse patient populations
- Ability to work with and communicate to team of providers
Pay: $85,000.00 - $150,000.00 per year
