Keene Valley Neighborhood House, a non-profit assisted living residence, is seeking a caring and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our healthcare team as a Nurse Care Manager. The ideal candidate will possess a passion for elderly care. This position is part of our management team and is mostly administrative with minimal hands-on care required. The schedule is Monday-Friday, 9am-4:30pm (37.5 hours per week including 30-minute paid lunch break). Join our team and work with an organization that cares about you and your family! Schedule Monday-Friday, 9am-4:30pm Qualifications - Current and valid LPN or RN license in the state of New York - Previous experience in long-term care or assisted living is preferred but not required. - Excellent communication and interpersonal skills. Responsibilities - Provides nursing care management to residents in accordance with their individual care plans. - Oversee medical technicians who administer medications and treatments as prescribed by a physician, and monitor residents for side effects or adverse reactions. - Reviews medication records daily. - Perform routine observational assessments, reporting any significant changes to the supervising nurse and/or resident’s physician. - Maintain accurate and timely documentation of resident care, including updates to care plans. - Be available for on-call consultations every other week (strictly by phone) – paid an additional $130/week Job Type: Full-time Pay: $60,000.00 - $80,000.00 per year Benefits:
