Position Description:
GENERAL SUMMARY
The Medical Caseworker will assist patients, visitors, and community residents in accessing and understanding the medical services provided by Cooper Green Mercy Health Services (CGMHS). Medical Caseworker will collaborate with healthcare professionals and community organizations to address needs of patients, focusing on care coordination, and resource referrals. Medical Caseworker will assist patients with various medical and social challenges, helping to improve their overall wellbeing. Furthermore, the medical caseworker will advocate on the behalf of patients to ensure their needs are being satisfied, while ensuring their access to healthcare services, financial assistance, and community resources are provided based on availability and eligibility. Perform any additional duties as set out by Cooper Green Mercy Health Authority, ensuring alignment with organizational goals and compliance with relevant policies and procedures. The Medical case worker will perform occasional duties outside of traditional work hours, such as participating in community events and volunteer opportunities. The Medical Caseworker will also be adhering to guidelines set forth by the Health Insurance Portability and Accountability Act (HIPAA), and the code of conduct set forth by Cooper Green Mercy Health Services Authority. Medical caseworker may also provide services to patients, non-enrolled patients, and community organizations.
KEY RESPONSIBILITIES:
- Assisting patients, visitors, and community residents with assistance in navigating and accessing medical and social services within our facility, as well as connecting them to external community resources.
- Provide guidance on navigating social services, such as housing, transportation, Medicaid, food assistance and mental health services.
- Educate patients and their families on healthcare processes, social services, and available resources.
- Respond to patient and community inquiries, offering information on services available at the Resource Center.
- Participate in cross-functional team meetings to discuss patient care, social services, and community engagement strategies.
- Support patients in coordinating follow-up medical appointments, referrals, and social service applications.
- Network with community partners and other organizations that provide financial assistance, resources, and other social services for patients.
- Resource center environment, with occasional visits to non-clinical and clinical settings.
- Aid in managing electronic resource board, completing non-clinical referrals, assisting patients with requests such as obtaining immunization records upon request. Directing non-enrollee patients to the enrollment department to assist with enrolling and providing additional navigation for resources.
- Collaborate with licensed Social Workers within the clinics of Cooper Green Mercy Health Services Authority to ensure patients’ social services needs are addressed.
- Report any barriers patients are facing with gaining access to their care, while discussing resources to alleviate these barriers with the patients and their families.
- Accurately document information regarding patients’ information as it pertains to the treatment plan and other information needed to assist the patients in their care.
- Administer assessments of patients that will assist in providing information that highlights the need for social services intervention.
