Job Summary
Neighborhood House has a longstanding commitment to ensure that low income, immigrant, and refugee aging and disabled members of our community age in place, receiving culturally appropriate services that enable the highest quality of life. The primary role of this position is to provide case management to approximately 20-30 clients and conduct in-home functional assessments and arrange for in-home caregiving for elderly and disabled adult clients receiving Department of Social and Health Services (DSHS) Medicaid funded personal care and other services. The Case Aide will work as a member of the MAC TSOA and LTC program’s administrative and clinical teams to provide data management, This position will report directly to the LTC admin Supervisor.
The ideal candidate will have experience navigating within a dynamic and flexible atmosphere and have experience working with diverse populations.
Responsibilities
  • Maintain a caseload of 20-30 clients; completing screening and assessments, home visits, and all other requirements to maintain program compliance with ALTSA Long Term Care Manual.
  • Utilize DSHS and ADS data systems to generate reports that monitor a number of revolving deadlines; communicate efficiently and effectively to clients, case managers, care coordinators and providers to help clients and their providers maintain compliance DSHS requirements.
  • With the support of the clinical team, coordinate the required documentation, authorization, and ordering process for medical equipment requests from clients in compliance with DSHS requirements. Develop relationships with Providers to improve the efficiency of this process.
  • Attend trainings and professional development activities as assigned; prepare trainings for the administrative and/or clinical teams as requested.
  • Provide language support to the team as assigned.
  • Translate flyers and proofread documents in your spoken languages as assigned.
  • Perform various administrative tasks, including managing e-fax communication, handling mail correspondence, document checking and monitoring phone calls.
  • Perform other duties as assigned.
Qualifications
Minimum Qualifications
  • Associate degree in social work, social sciences, health, human services or related field. Experience accepted in lieu of degree.
  • Two years of experience working in social services, health, or a similar high-paced environment with an emphasis on diverse populations. An equivalent combination of education and experience may be considered.
  • Demonstrated ability to perform job duties within the context of complex Federal and State regulations and definitions and to complete extensive technical documentation.
  • Very organized with a high level of attention to detail.
  • Demonstrated ability to provide supportive customer service in a high-paced environment.
  • Ability to communicate effectively, both orally and in writing, with individuals from a wide range of cultural backgrounds.

  • Must have reliable, independent transportation for periodic travel to client homes and between work sites.
  • Must be proficient in key computer applications (MS Office/Word/Excel/Power Point/Access).

Desired Qualifications
  • Bachelor’s degree in Social Work, Behavioral or Health Sciences or related field.
  • Knowledge of and proficiency in client services-tracking database or related database navigation.
  • Knowledge of and proficiency in case management in a multicultural setting
  • Knowledge of and experience in elderly or disability issues and social or health related resources that assist elderly and individuals with disabilities maintain their independence and quality of life.
  • Bi-lingual in Spanish
Required Credentials
  • Must pass Neighborhood House background check requirements.
  • Position requires Washington State bi-annual criminal background re-check.
Post-Hire on-the-job Training Requirements
  • Current First Aid Certificate within 30 days of hire.
  • Current CPR Certificate within 30 days of hire.
  • Sexual Harassment Training within 30 days of hire.
  • Child Abuse and Neglect training (within 2 weeks of hire).
  • HIPAA training within 30 days of hire.
  • CARE Training within 90 days of hire.
  • Case Management Program Training within 1 year of hire.
