Case Manager

  • Reach out to clients assigned by his or her supervisor, assess their most urgent needs, appraise the situation, and listen to their concerns.
  • Managing a portfolio of cases.
  • Working in accordance with relevant legislation, healthcare best practices, and internal policy and procedure
  • Perform an in-depth mental and/or physical health analysis of the client.
  • Develop a detailed plan of action to meet the client’s needs, set goals, and find the necessary resources to meet those goals.
  • Offer counseling to patients in either individual or group settings.
  • Consult with external agencies in order to provide clients with support services and resources.
  • Keep comprehensive records of clients’ progress throughout the process, including every call, referral, and home visit they make. Provide weekly reports to the Medical Director listing patients being investigated and their disposition.
  • Maintain the confidentiality, respect the privacy, and preserve the routine and independence of clients as much as possible.
  • Stay in touch with clients to ensure the services were beneficial, and that their needs are still met after they’ve been pointed in the right direction for their problems.
  • Work with each service provider on discharge planning.
  • Helping to manage patient healthcare needs and services in areas such as long term care , mental health, substance abuse and geriatric care.
  • Contributes to supporting the mission of STEEMCC by ensuring that quality patient care is provided in each patient contact and during each clinical encounter
  • Other related duties and responsibilities as designated by the Medical Director
Job Type: Full-time
Pay: $53,000.00 - $60,000.00 per year
