Admissions Nurse Home Care

We are looking for a PT Registered Nurse to join our Cary team. What does a Home Care RN do????
A Home Care RN focuses on overseeing and coordinating non-medical care for patients, particularly those who need assistance with daily living. In this role, the RN does not perform complex medical procedures but still ensures that patients receive the appropriate support and care in a safe, healthy home environment. Here are the key duties of a Non-Skilled Home Care RN:
1. Care Coordination and Supervision:

Oversee and supervise non-medical caregivers, home health aides (HHAs), or personal care aides (PCAs) who provide direct support to clients.
Ensure that caregivers are following the care plans and delivering high-quality assistance with daily living tasks.
Conduct home visits to assess the quality of care and ensure that client needs are being met.

2. Create and Update Care Plans:

Develop and regularly update personalized care plans based on the needs and preferences of the patient.
Care plans often focus on non-medical support, such as assistance with personal hygiene, mobility, meal preparation, and household tasks.

3. Patient Assessments:

Conduct initial and periodic non-clinical assessments of the patient's physical, emotional, and social well-being.
Evaluate the patient’s living conditions and ensure that their home environment is safe, clean, and conducive to their well-being.

4. Assist with Activities of Daily Living (ADLs):

Supervise and ensure that caregivers assist with ADLs, such as bathing, dressing, grooming, toileting, and feeding.
Help develop strategies to promote the patient’s independence with ADLs as much as possible.

5. Provide Family Education and Support:

Offer guidance to families on how to assist their loved ones with daily living activities and how to create a safe and comfortable home environment.
Act as a resource for family members who may need advice on non-skilled caregiving techniques.

6. Non-Medical Caregiver Training:

Train non-medical caregivers on basic safety and care techniques, such as fall prevention, safe transferring, and infection prevention.
Provide ongoing education to improve the quality of care delivered by non-medical staff.

Qualifications:

Current and valid RN license in the state of North Carolina.
Previous experience in home care, geriatrics, or a similar setting is preferred.
Strong communication and organizational skills.
Ability to work independently and as part of a team.
