Summary
The Registered Nurse coordinates and oversees health and medical services in accordance with regulatory, accreditation, and agency standards for children with severe emotional disturbances in our new innovative community-based Child Focused Assertive Community Treatment Team. Child ACTT is a team-based multi-disciplinary approach to serving kids in their homes, kinship placements, foster homes, or may begin during a transition from a more restrictive residential setting or hospitalization. Child ACTT is primarily community based, however some flexible home and office-based work will be required. This is an exciting new service in North Carolina, with the goal of helping children with severe emotional disturbances stay in their home environments successfully.
Principal Duties and Responsibilities
Specific duties and responsibilities:
Nursing Treatment and Assessment
  • Assume primary responsibility for medication administration management and coordination
  • Assist in the assessment and admission process by evaluating prospective clients with a concentration on past medical history, current physical and mental status, and organizations to provide appropriate medical care while in services at Children's Hope Alliance, compiling historical data and updating as needed
  • Conduct on-going assessment of medical needs throughout course of treatment
  • Monitor client medical status while in placement, managing medical, dental, and chemical interventions, including assessment, vital signs as indicated, and follow-up care
  • Explain medical procedures and treatments to clients to gain understanding and cooperation, and to ensure the information, comfort, and safety needs of clients are met
  • Initiate client and family health and wellness education plan according to individual needs of the client, including consideration of discharge planning
  • Attend meetings when medical input is beneficial or necessary (including daily/weekly staffing, medical rounds, admission staffing, etc.)
  • Guide organizational response to staff and client medical emergencies, responding to life-saving situations based upon nursing standards, policies, procedures, and protocol
  • Coordinate, conduct, assess, and document regular trainings on medical and health-related issues.
  • Facilitate education on appropriate content applicable to care - ex. hygiene, health-related topics, etc.
  • Participate in 24/7 on call rotation.
Team Management
  • Provide oversight of medical services provided and recommendations during the course of treatment
  • Serve as liaison with physicians and psychiatry to assure that appropriate medical care is provided
  • Consult regularly with involved psychiatrists or other medical providers, relaying observations regarding clients' condition and reaction to drugs, treatments, and significant incidents, keeping appropriate individuals apprised.
  • Participate as a member of a clinical team and organization as directed and in cooperative, client-focused manner through daily/weekly client staffing, team meetings, scheduling of work days, and participation in training events.
  • Monitor compliance and documentation of lab work, genesight testing, and annual wellness exams
  • All other services as assigned.
Administration
  • Provide written medical reports as necessary
  • Maintain appropriate records including medications administered, client response, side effects
  • Coordinate and monitor staff medication administration cycle; when applicable
  • Participate in developing and maintaining medical policies and procedures appropriate to current and anticipated client treatment needs, as per licensure and accreditation standards
  • Maintain quality assurance measures on medication administration documentation and procedures to ensure proper distribution of medication in accordance with approved nursing, licensure, and accreditation standards

Requirements: Education and Experience Requirements
  • Associate's degree (A. A.) or equivalent from two-year college or technical school
  • At lease one years' related experience and/or training
  • North Carolina Licensure as a Registered Nurse
  • Must possess basic computer skills and be able to utilize an electronic health record and associated software and hardware.
  • Must have a valid Driver’s License.
  • Must submit to and pass pre-employment drug screening, criminal, HCPR, OIG, EPLS and driving checks.
  • Must submit insurance and registration documentation if utilizing personal car for work related transports.

  • Physical functions which are considered essential to the satisfactory performance of the job include the following: reading, typing, writing, speaking, and using the telephone and prolonged sitting at a desk and working on a computer.
  • Employee must be able to operate a vehicle for job duties, and keep a valid NC drivers license with insurance.
  • The work environment will contain slight to moderate office-related noises. The employee is not exposed to extreme weather conditions, toxic fumes, or airborne particles.
  • The employee must occasionally travel to different locations in the course of work.
  • Physical functions which are considered essential to the satisfactory performance of the job include the following: public speaking, reading, typing, writing, using the telephone, driving, therapeutically restraining children and adolescents in crisis situations.
