JOB DESCRIPTION
We are recruiting Project Staff to support the running of Y Gelli. The working hours are on 'an as and when' required basis to assist in the covering of staff absences or when additional support is needed above the usual staffing compliment. All new starters are required to register or hold registration with Social Care Wales and will have a full induction along with the necessary training.
JOB BACKGROUND
The establishment called Y Gelli is the result of a joint initiative between Vale of Clwyd Mind Association, who take lead responsibility for the management, Grŵp Cynefin (Housing Association) and the Betsi Cadwaladr University Health Board. Y Gelli is registered as a care home with Denbighshire County Council. It consists of six self-contained flats, providing 24 hour individual support to ten people who experience severe and long term mental health problems, and as a result have continuing care and housing management support needs.
Y Gelli specifies close linking with the Community Mental Health multi agency teams. Within the units, individual programmes of support and rehabilitation are carried out with common themes of increasing daily living skills and the ability to make choices and decisions, thus improving the quality of life and range of life experience available to the residents. The relatively small size of Y Gelli makes the development of closer relationships and friendships, both with other residents and people in the local community, a valuable and valued experience which is not possible either in large hospitals or in isolated single dwellings.
PROJECT WORKERS ROLE
To be registered with Social Care Wales
To ensure that the physical, emotional, social, and intellectual needs of the residents of Y Gelli are met. Close attention should be paid to individual needs, and particular emphasis laid on participation and choice in all aspects of daily living.
To support the residents in activities which enhance the process of community interaction in accordance with the wishes of each individual resident.
To take responsibility for the smooth day to day running of the home in the absence of the Manager and or Senior Project Workers. Support will be available from the “on-call” Senior staff to use for help and advice, including out of hours support
ACCOUNTABILITY
The Project Worker is accountable to the Manager, or the Senior in his/her absence.
RESPONSIBILITIES
1. To ensure that the values and procedures incorporated within the Statement of Purpose of Y Gelli are upheld.
2. To enable individual residents to care and support themselves to their best ability, and assist residents with all areas of need, as identified in individual support plans.
3. A key working system is in place within the home. You will be responsible for ensuring residents’ needs are assessed, planned, implemented, and evaluated in conjunction with the residents. This involves the drawing up of care/support plans.
4. To offer individual support, always working closely with other staff involved in order to ensure a consistent approach. Senior Project Workers are available for advice.
5. To report on the effectiveness of developmental programmes to the Manager or Senior.
6. A flexi system of working has been introduced to enable staff to utilise time with residents, and staff will be expected to comply with this philosophy e.g. day trips, overnight stays, holidays. For this reason, it is essential to have a car.
7. Accompany residents shopping and to appointments, e.g., clinic appointments with doctors, chiropodist, and dentist.
8. To participate in the maintenance and the cleanliness of the home and individual residents’ flats. You will be expected to encourage and support residents in this area.
9. To provide written reports as requested by the Manager or Senior, e.g., residents’ six-monthly progress reports and review minutes.
10. To be responsible for the provision and maintenance of good standards of care and support as identified and delegated by the Managers.
11. To assist in the supervision of residents’ self-medication, and during illness, accident or emergency, taking the necessary action to obtain medical advice and notify next of kin when appropriate.
12. To encourage and foster good relationships between staff and residents, developing as far as possible a group spirit.
13. To establish and maintain good relations with neighbours and members of the local community and community agencies.
14. To establish and maintain good relations with relatives and friends of the residents.
15. To be approachable in your manner in order to develop and maintain trust from both the staff team and residents.
16. To achieve and maintain effective working relationships with colleagues, both in and outside the Organisation. In so doing you are expected to promote a positive, professional image of the Organisation.
17. To respect confidential information in line with Vale of Clwyd Minds policies and ensure the safe keeping of written records in accordance with the Statement of Purpose of Y Gelli.
18. To take responsibility of the smooth day to day running of the home in the absence of the Manager and or Senior Project Workers and ensure staffing cover meets with the minimum requirements of the Regulation and Inspection of Social Care (Wales) Act 2016 (RISCA).
19. Assist residents to manage their own financial affairs, and give necessary support as required. You will be expected to keep accurate financial records in line with Policies and procedures and if discrepancies are found bring them to the attention of the management
20. Y Gelli management have a commitment to training, and feel it is a vital part of development of an effective working team. Therefore, training will be made available to Project Workers if required (subject to staffing levels and funding). It would be expected the Project Worker will have had experience of working within the mental health field and be trained or have a willingness to work towards a relevant qualification (NVQ III) in the mental health and support field.
21. To take on responsibility for key areas as required in negotiation with the Manager in a view to developing your skills and role.
22. To familiarise and adhere to all Vale of Clwyd Mind’s Policies and Procedures.
23. To take reasonable precautions to ensure the safety of the residents in and out of Y Gelli.
24. To be aware of current Health and Safety legislation in relation to employees responsibilities e.g., fire, accidents, risk assessment.
25. To undertake, and encourage the effective and economical use of gas, electricity, heating, and telephones, etc.
26. To participate in sleep in duty to ensure 24-hour cover is provided to residents.
27. Other duties as specified by the Manager or Senior compatible with the post.

CONDITIONS OF SERVICE
The Project is staffed on a 24-hour basis in accordance with a shift system, and all Care staff will be required to work within the system including sleep in duties.
It is a requirement to be registered with Social Care Wales have a car for use to carry out the duties of this post.

Pension
A 5% contributory company pension scheme is available for permanent Vale of Clwyd Mind staff. This is administered by TPT formerly The Pensions Trust.
Notice
Four weeks’ notice will be required on termination of employment.
Smoking Policy
We wish applicants to be aware that Y Gelli is not a totally smoke free environment.
Salary Points
£13.69 per hour.
This post is subject to a satisfactory medication examination by the Occupational Health Department and an Enhanced Disclosure application to the Disclosure and Barring Service.
The management reserves the right to alter staff Conditions of Service in agreement with the individual staff.
