Job Summary:
We are seeking a dedicated and compassionate Housing Support Worker to join our team. The housing support worker is specifically focused on assisting individuals in achieving increased housing stability and self-sufficiency by assessing for services and implementing the services based on the individuals’ needs and goal plans. This role requires strong communication skills, a thorough understanding of community supports and social assistance programs, and the ability to work effectively within a multidisciplinary team.
Responsibilities:

• Providing case management and outreach to homeless individuals;
• Conducting assessments for program participants and developing and monitoring individual support plans to address problems/concerns;
• Providing advocacy and community referrals of service;
• Communicate move-out dates and establish move-out plans that oversee and support in their transition;
• Developing and maintaining client data and records for all participants.

Qualifications/Skills:

• Bachelor’s degree in social work or related human services field is preferred.
• At least 2 years of direct services experience working with high risk populations and/or special needs populations that are culturally diverse.
• In lieu of a Bachelor’s degree, must have at least 6 years of housing case management or closely related work experience. Previous experience working in an urban community and nonprofit environment is a plus.
• Qualified applicants must have a valid driver’s license, must have a good driving record and be able to transport individuals as needed (company vehicle can be available when needed). Criminal record & vulnerable sector check required for this position.

Compensation is dependent on qualifications and experience.

Pay: $45,000.00-$60,000.00 per year
