Position Summary
The Community Disability Services Worker is a front-line role that supports the development of skills and relationships that help persons with disabilities work towards and meet their goals. CDSW upholds human rights, health, safety and well-being of all people in service and colleagues.
Essential Responsibilities
  • Carry out quality of life plans to help the individuals we serve with their personal development, community involvement, behaviour management and/or personal care.
  • Help individuals participate meaningfully in setting personal goals and planning the steps needed to reach them, and in expanding opportunities for personal growth and purposeful roles within the community. This includes following the individual’s and/or the family’s desired direction in supporting goals and wishes as much as possible.
  • Help individual’s make and maintain shared, respectful relationships with others in the community to support meaningful participation and inclusion. This includes participation in employment, volunteer, recreational and other social settings
  • Understand community opportunities and issues to help the individual access and participate in community life.
  • Model socially-appropriate behaviours (for both the individual and the community) and assist and encourage the individual to make and maintain appropriate community connections. This may sometimes include acting as a support ‘coach.’
  • Support and advocate for community members and other service providers to demonstrate a non-judgemental welcoming approach. Influence the delivery of sensitive support and appropriate accommodations that meet the individual’s needs.
  • Be aware of the individual’s physical, social and recreational environments, and act to minimize risks and maximize participation and inclusion. When unsure, ask for help from more experienced people.
  • Be aware of and follow policies, practices and operational protocols which includes knowing and following medical protocols and standing orders for medication and other remedies.
  • Help individuals achieve their goals by developing basic life skills, build their confidence, competence and participation within their community networks. This may include modelling desirable behaviours for life skill development and (particularly in the case of service to children) academic skill development.
  • Encourage and help individuals to advocate on their own behalf, when required to have their needs met; and, when appropriate, support the individual’s desires, goals and life choices by interacting and advocating with the individual’s social network.
  • Create clear and objective records, required reports, and other essential documentation.
  • Communicate in a professional, timely, accurate, and respectful way with individuals, families, guardians, staff, and community members.
  • Work effectively in a team-oriented environment.
Essential Skills
  • Carry out Quality of Life Plans and read/complete daily documentation in My Compass.
  • Demonstrate knowledge of quality of life plan definitions, actions and measurement, and how human needs and social determinants of health influence service delivery.
  • Actively participate in Human Centered Design.
  • Report and complete incident documentation.
  • Attend and participate in all required meetings in a professional manner (i.e punctuality, engagement, communication and problem solving).
  • Complete and maintain certification in mandatory training.
  • Demonstrate knowledge and proficiency of support procedures (Quality of Life Plan, Planned Positive Procedure and Restrictive Procedure).
  • Accurate completion of payworks entries and mileage expense claims.

Your Qualifications
  • A High School diploma with a certificate in disabilities studies preferred
  • 6+ months of relevant work experience
  • Proficient word processor, specifically Google Doc, Sheets, Slides, Calendar and Email
  • Effective time management and priority-setting skills, and the ability to balance competing demands/priorities and deal with competing opinions.
  • Excellent written and verbal communication skills are essential.
  • Valid Drivers License and clear drivers abstract preferred.

Pay: From $19.65 per hour
Expected hours: 37.5 per week
