About the role
The purpose of this position is to provide specialised clinical and consultancy services of the Voluntary Assisted Dying scheme across West Moreton Health. The position delivers advanced clinical leadership, case management and care coordination services to stakeholders and consumers within WMH and provides education, development, and workforce support to achieve excellence in patient care.
The CNC (VAD Connect Service) provides contemporary leadership in clinical services of voluntary assisted dying that is innovative and encourages participation in treatment and care aimed at achieving person-centred outcomes through the application and role modelling of professional nursing practice standards in accordance with the Nursing and Midwifery Board Australia (NMBA). This role practices within the regulatory framework prescribed in the Queensland Voluntary Assisted Dying Act 2021.

- Advanced level clinical expertise and high-level capability in the application of high quality, patient centred Voluntary Assisted Dying services.
- provide collaborative care and engaging in reflective practice that positively impacts organisation and patient outcomes
- practice that is evidence based and continually monitors and evaluates nursing activity in the advance care planning specialty area
- contribute to an effective nursing team that works collaboratively with colleagues, patients and their carers.
- Has a minimum of 5 years senior nursing leadership clinical expertise, including leading teams through change. Working towards post graduate studies, whilst not mandatory would be advantageous
- Demonstrate a high standard of clinical person-centred care. Accountable for own competence and clinical knowledge; utilises effective assessment, planning and implementation skills to deliver and evaluate safe, quality person-centred care including critical thinking and decision making.
- Demonstrate knowledge of commitment to quality processes including policies, guidelines and evidence-based practices. Participate in incident management resulting in patient safety and quality improvements.
- Demonstrate ongoing learning and professional development to provide safe, quality person-centred care including health education to patients and others.
- Demonstrate the application of evidence-based guidelines to achieve positive patient outcomes and identify areas of improvement.
- Demonstrate clinical leadership in the coordination and provision of patient centred care and best practice outcomes.
- Demonstrate collaborative teamwork and decision making, empathy, integrity and respect for all patients, carers and staff.
- Willingness to become an authorised Voluntary Assisted Dying (VAD) Administering Practitioner including mandatory training to administer the voluntary assisted dying substance to a person who has chosen practitioner administration. - Willingness to undergo additional training (if necessary) to ensure competency in cannulation.

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
