Job Vacancy: NDIS Support Coordinator

Position Overview: We are seeking an NDIS Support Coordinator to join our team. In this role, you will empower NDIS participants to achieve greater independence by assessing their unique needs and identifying the services and supports required to help them reach their goals. You will collaborate closely with participants to coordinate their support, ensure they make the most of their NDIS plans, and overcome any challenges by developing and implementing effective action plans.
Responsibilities: As a Support Coordinator at Medika Care & Support, your role is a dynamic mix of proactive problem-solving and strategic planning. You’ll begin each day by reviewing emails and prioritizing tasks to ensure timely responses. Every day brings new challenges, from managing budgets and addressing service quality concerns to negotiating with providers and advocating for your clients’ needs. Regular meetings with clients and their care teams will allow you to adjust to their evolving needs and keep their plans on track. Your expertise in NDIS processes and legislation will be key in guiding clients through plan adjustments, with the flexibility to handle urgent matters as they arise. Balancing administrative tasks with fieldwork, you’ll directly engage with clients and stakeholders, promoting a holistic approach to service coordination. While you’ll work independently, collaboration is essential, requiring creative thinking to research and propose new opportunities that enhance your clients' lives.
Support Connection & Coordination of Support: Provide Level 1 Support Connection and Level 2 Coordination of Support for NDIS participants, ensuring seamless access to services and effective coordination of support plans.
  • Stakeholder Liaison & Relationship Building: Build strong relationships with key stakeholders, including other NDIS providers, community venues, community partners, and allied healthcare teams, and foster open communication and collaboration to enhance participant outcomes.
  • Budget Monitoring & Expenditure Management: Monitor plan outcomes and expenditures to ensure that services align with participants' plan budgets. Implement strategies to optimise resource allocation while maintaining the quality of support services.
  • Caseload Management: Independently manage a varied caseload of NDIS participants in the community. Ensure the provision of high-quality care, meeting client objectives, and addressing participant requirements effectively.
  • Initial Assessments: Conduct thorough initial assessments of participants to identify their needs, goals, and preferences. Gather essential information to develop comprehensive support plans tailored to individual requirements.
  • Referral & Research: Research and identify relevant services within the community, and refer participants to appropriate resources when needed. Focus on supporting participants in their recovery journey by connecting them with suitable services and opportunities.
  • Advocacy: Advocate for the rights and interests of clients, ensuring they have access to appropriate services, resources, and opportunities.
Skills & Experience:
  • Minimum of 2 years experience in Support Coordination within the Northern Territory.
  • Experience in one or more of the following: Support Coordination, Support Connection, Local Area Coordination, NDIA Planning, Mental Health, Welfare Work, or Counselling.
  • Comprehensive knowledge of the National Disability Insurance Scheme (NDIS) and its operational processes.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time-management capabilities.
  • Tertiary or Diploma-level qualification in a relevant health or welfare discipline, such as Social Work, Social Science, Community Services, Case Management, Mental Health, Psychology, or other Allied Health fields.
  • NDIS Worker Screening Check.
  • Valid Driver’s Licence.
