About the Role
We are seeking a motivated CHSP Coordinator to coordinate and manage services for people living at home through the Commonwealth Home Support Program (CHSP).
In this role, you will conduct assessments, create care plans, and oversee the delivery of services that support our clients’ independence. You will act as the first point of contact for inquiries, collaborate with healthcare professionals, and ensure compliance with Aged Care Quality Standards.

About the Role

Key Responsibilities
- Coordinate service delivery and manage client care plans
- Conduct assessments and complete documentation
- Build strong relationships with clients, families, and providers
- Ensure compliance with industry standards and foster continuous improvement
- Tertiary qualification in Health, Disability, or related field (or equivalent experience)
- Knowledge of CHSP, Home Care Packages, and Aged Care Standards
- Strong organizational and time management skills
- Advanced computer proficiency (Microsoft Office)
- Excellent communication and relationship-building skills
